Dislocing health issues to employer?

I have just joined a company in London and am coming to live in the UK with my fiancee from Holland. If I understand correctly, we are covered by the NHS and I also have private health coverage paid by my employer. My fiancee has some past health problems she doesn’t want to devulge. I suspect we will have to fill in a form for the insurance. Is it in obligation to tell the employer about her health issues or can we keep this for ourselves? If we do tell the insurance company, do they than tell my employer or is this considered private information? I would like to keep this info from my employer as it is quite private and doesn’t concern them directly. Many thanks for any insight.
